Collect Key documents needed for submission of the application.

If you do not have all the documents for the application, you are able to return to your application.

US Domestic Students:

On Campus

• Insurance Information (if none, student must sign a waiver)
• Immunization records (childhood and adolescent immunization - not CoVid shots)
• High School Diploma / GED
• High School Transcript(s) – Use the application request process. English translation and evaluation may be required.
• ACT/SAT/Accuplacer official scores and documentation (Test takers name and date must be visible)
• Any College transcripts – Indicate the number of institutions and use the application request process. English translation and evaluation may be required. The sending institution will transmit the transcripts directly to admissions@champion.edu
• Two references – provide email address for two references (non-family or guardian)

Online

• High School Diploma or G.E.D.
